Clopidogrel – A Key Antiplatelet Drug for Heart and Stroke Prevention

 

Introduction:


*Clopidogrel* is a widely prescribed *antiplatelet medication* used to *prevent blood clots* in individuals at risk of *heart attack*, *stroke*, and other cardiovascular events. It belongs to the *thienopyridine class* and is often used in combination with aspirin in dual antiplatelet therapy (DAPT), especially after *stent placement* or *acute coronary syndrome (ACS)*.


Mechanism of Action:


Clopidogrel is a *prodrug* that is converted in the liver to its active metabolite. It works by *irreversibly inhibiting the P2Y12 ADP receptor* on platelets, preventing their activation and aggregation, thereby reducing the risk of thrombus formation.

- *After stent placement (PCI)*  

- *Peripheral arterial disease (PAD)*  

- *Acute Coronary Syndrome (ACS)* in combination with aspirin


Contraindications:


- Active pathological bleeding (e.g., peptic ulcer, intracranial hemorrhage)  

- Known hypersensitivity to clopidogrel  

- Severe hepatic impairment

- *Thrombocytopenia*
